Ordering Transcripts - Process for Current Students

SECONDARY SCHOOL REPORTS AND APPLICATIONS
For Students Applying to Private and Out of State Universities

In addition to your application, many private schools will require you to submit a Secondary School Report (SSR) or Counselor’s Report (CR) which you will receive with your application. This form must be filled out by a school official and mailed from the school. Also, many out-of-state universities will require a school official to fill out and sign a portion of the application. The following steps should help you in compiling the necessary documents:

  1. IMPORTANT: Check your DUE DATE. Make sure you give each person involved at least TWO WEEKS time to complete your documents.

  2. Fill out the applicant’s portion of the SSR or application and turn it into me along with a Transcript Request form for an official transcript that is available at my desk. Transcripts cost $3.00 each. The transcript must be mailed with your SSR.

  3. You must also submit to me a stamped, legal sized envelope addressed to your college. Include enough postage on your envelope to mail 4 pieces of paper plus your SSR (approximately $1.00) You may use a large manila envelope if you like, but it is not necessary. (See sample below)

  4. Clear all your debts. If you have a debt, your transcripts will not be mailed.

  5. Your counselor will enclose a school profile that gives an overview of MSJ and explains our grading system and your official transcript that includes courses in progress. Advise me of the due date and I will mail this information by that date.

Please plan ahead.

Reminders: Students must:

  1. Fill out a Transcript Request form

  2. Clear all debts

  3. Pay $3.00 each per transcript fee.

  4. Submit a stamped, legal sized, envelope addressed to your college

  5. Any incomplete requests, will be placed on hold and will not be processed and will be returned to you.

 

LETTERS OF RECOMMENDATION

Private universities usually require 1 or 2 letters of recommendation. UC’s and CSU’s do not need letters. Letters of recommendation should be written by those teachers who know you best. Because of the large number of students at MSJ, administrators and our counselors usually do not write letters of recommendations. Teachers who have worked with you daily are your best choices. Keep in mind when asking for a letter of recommendation that the task of writing for you is a courtesy, not a duty. Be sure to THANK the teachers!!

  1. IMPORTANT: Allowing your teacher enough time is very important. Remember, you need to allow at least two weeks for your teachers to complete your letters of recommendation.

  2. Help teachers write your letters by filling out the attached Letter of Recommendation Form.

  3. Make copies of your Recommendation Form so you only need to fill it out once.

  4. Give a copy to each teacher you are asking to write a letter.

  5. Give your teacher a STAMPED, ADDRESSED ENVELOPE. Teachers must mail your letters of recommendation. They will not be returned to you. You may enclose a copy of the attached letter with your application explaining MSJ’s policy. Also, have enough postage on your envelope to include all your paperwork.

  6. Writing letters is a lengthy task. Do not ask a teacher to write unnecessary letters. You may not compare letters to send the better one. If your selected teacher will not be able to write your letter on time, ASK ANOTHER TEACHER PROMPTLY.

MID-YEAR REPORTS

For those schools requiring a Mid-Year Report, you should submit the following to me in January:

  1. Fill out a Request for Transcript Form. This transcript will include your 1st semester grades.

  2. A stamped addressed envelope with enough postage for 3-4 pieces of paper (two .39¢ stamps should be enough). Please refer to the sample on the reverse side of this page.

  3. The Mid-Year Report form and the application should be filled out.

I will mail the completed Mid-Year Reports in February, as soon as the new transcripts are printed.

FINAL TRANSCRIPTS

All schools will require a Final Transcript to include your 2nd semester grades and your graduation date. Order these in May, again, supplying me with a stamped, addressed envelope with enough postage for 1 piece of paper).
